The project goal is treatment success for all patients, regardless of income. Successful treatment often requires patients to have an extensive support system beyond the resources obtained through health care providers. Someone without such a support system may be overwhelmed by the enormity/complexity of treatment and may choose not to access care and treatment may not succeed because of these complications. Goal is to serve a minimum of 50 over a one-year period. Services include: transportation to appointments, in-home support services, advocacy, home delivered food; and one-on-one emotional support.

Emergency Financial Assistance Program
The Breaking Barriers Emergency Financial Assistance program will set up an emergency fund specifically to insure that no woman that is newly diagnosed or is being released from the medical system will have to live in her car or on the streets immediately after surgery and to assist women on the verge of homelessness to stabilize their living situation. The assistance is time limited, and during the process of accessing this service their staff will assist the women in accessing housing programs and benefit programs to ensure they are stabilized in addition to the rental assistance.

Be A Voice for Wellness
This program provides breast cancer education for the Native American community in Butte County. The “Be a Voice” program utilizes a Native American traditional Talking Circle combined with instruction in the traditional arts to yield important information on barriers to completing a breast cancer screening and to deliver life-saving breast cancer early detection information in a culturally effective manner. They plan to reach 150-250 women and conduct follow-up to ensure that a minimum of 45 eligible Native American women enroll in the CDP: Every Woman Counts program and receive a free breast screening.

Patient Navigation and Support Project
This Project serves uninsured, low-income symptomatic and diagnosed breast cancer patients, who reside within the Sacramento Valley Affiliate service area. Services are provided via a toll-free number. An English-Spanish bilingual Care Coordinator provides Project participants with emotional support, linkages to and coordination with a variety of financial and supportive services, guide in securing coverage for treatment, and education materials. Project funding assists with transportation to medical appointments, and for diagnosed patients, home/personal health support needs, such as utility payments and grocery store food vouchers.

Breast Cancer Screening, Education, Outreach and Case Management
This program provides screening to low-income, uninsured and underinsured women in Yolo County, many of whom are further challenged by language and cultural barriers and have had little or no access to preventive health care services. Educational information will be provided and includes bilingual information about breast examinations and mammography, locations of CommuniCare’s four primary care clinics in Yolo County, referrals to mammography services, and assistance in coordinating transportation to clinics and appointments. Anticipates serving a minimum of 1,000 women.

Breast Cancer Education, Screening and Early Detection
This program provides low-income, uninsured and at-risk women between the ages of 14 and 39 with breast health education, as well as early detection and diagnostic services for breast cancer. Delta will continue to increase it’s breast health education and outreach efforts through classroom presentations at local high schools. Covers Stockton and Lodi areas.

Expanded Outreach to Underserved Women Age 39 or Younger
This project will integrate breast health education and promote no cost screening and diagnostic services available to low income women age 39 or younger residing in the Yuba, Sutter and Butte counties. 500-1000 women will be reached with breast health information and 60-70 women with breast screening services. Three new clinics were added to expand outreach.

Hmong Breast Health Program
This program goal is to provide awareness and education on breast health and cancer, distribute translated materials, create a public service announcement in Hmong and air on Hmong radio, and provide navigation of breast examinations and mammograms to Hmong women in the Sacramento region.

Mujeres Informadas
This program proposed to increase breast screening in the Westside community of Stanislaus County by targeting 3 service areas where they is a high prevalence of medically underserved women. This innovative program will recruit, train and mentor women in these communities to serve as Promotoras who will provide health education, resource information, health assessments, referrals and support regarding breast health to women in their communities.

Molecular Breast Imaging
Positron Emission Mammography (PEM) is a high-resolution PET scanner dedicated to breast imaging which can detect 1.5-2.0 mm breast lesions. PEM is a molecular imaging test that can more specifically detect cancerous breast lesions than those seen in MRI which are sensitive for the detection of anatomic abnormalities, but are less specific. Preliminary findings demonstrate that PEM helped physicians manage their patient’s disease more accurately. Now we need to include a broader analysis of data so we can demonstrate a cost benefit to payers while still providing the care to the underinsured. Since PEM is particularly helpful in women with dense breasts, fibrocystic disease and implants, where conventional imaging can be non-diagnostic, PEM can be made available to those women who are high risk for breast cancer. Today, these women are at risk of not having their breast cancer diagnosed as early as possible. Target of 250 scans.

Peer Support Services & Education for Spanish, Bi-lingual and English speaking Breast Cancer Patients in the Sacramento Valley
This program provides breast cancer peer support and breast health education to patients in the Sacramento Valley. Anticipates serving 100 breast cancer patients in their support groups. 1) Will provide 6 bi-monthly continuous breast cancer support groups led by breast cancer survivors; 2) medical interpretive services; 3) transportation support to Spanish breast cancer patients; and 4) a Spanish peer support 24-hour breast cancer hotline. Save Ourselves will also distribute 1,500 pieces of Spanish breast health educational materials in outreach efforts.

“Check It Out” Teen Breast Health Awareness and Education
This program provides breast health awareness and education in an assembly format (six) directed toward young women and high school female student to bring awareness of the importance of breast self-examinations/general breast health. Anticipates serving 1,000 women.

Breast Cancer Navigation Program
SNMH’s Breast Cancer Navigator Program serves approximately 950 patients each year in Western Nevada County. The Navigator begins support and guidance even before the patient is diagnosed. Annual screening reminders and follow-up calls will start the communication. The Nurse Navigator will discuss the journey that the patient is about to travel and give the patient educational material. The Navigator acts as an advocate for the patients as well as a liaison between physicians. In addition, the Nurse Navigator continues to be a primary contact for the patient.. The goal of expanding this program is to ensure the best quality and continuum of care for our breast cancer patients as well as to ensure the future of this program.

Multicultural Breast Cancer Education and Outreach Program
This program will use culturally sensitive and linguistically appropriate methods to assist a minimum of 996 underserved women over the age of 40 residing in the service area of the Sacramento Valley Affiliate of Susan G. Komen for the Cure to obtain mammography services as recommended by the American Cancer Society. We will also provide breast health education to 15,000 women during the year covered by this proposal.

Breast Cancer Navigation Program
This program serves as a resource for breast cancer patients treated at Sutter Cancer Center as they navigate their way through the process of breast cancer diagnosis and treatment. Proposed enhancements will provide additional information regarding surgical options, care and recovery to women newly diagnosed with breast cancer and who speak Spanish, Russian and Chinese. Ten underserved women who are at risk for, or who have developed lymphedema will receive financial assistance to purchase treatment related supplies.

Early Breast Cancer Screening and Case Management Project
This program provides clinic based education, risk assessment and clinical breast exams as well as no-cost mammograms, diagnostic testing, and case management services to low-income women (age 20-39) in Sacramento County who are at high risk for breast cancer. Onsite child care and transportation to medical appointments may also be provided. Anticipates serving 200 women.

Healthy Breast Program: Consortium of Student Run Clinics
The Consortium is a collaboration of five established clinics serving low-income neighborhoods in Sacramento. The Consortium brings each of the clinics together to share best practices, collaborate on policies, procedures and common data collection and coordinate the solicitation of donations, pharmaceuticals and equipment. The Consortium will maintain the healthy breast patient survey, supervise Project Coordinators and Promotores for each clinic, maintain a common database, provide mammogram tracking/reminder reports, compile and compare data from each clinic regarding success rates for outreach, screening, mammograms, referral practices and outcomes and identify best practices.

VN Cares Healthy Breast Program
Utilizing a Promotores model of healthcare outreach, this program will provide healthy breast education, awareness and screening to 500 primarily Asian American/Pacific Islander women served by the organization and assure that 200 women receive clinical breast examinations, mammograms and the necessary follow-ups and treatments.

A Day for You, Rural Breast Health Awareness and Screening Program
This program offers clinical breast exams, mammogram screening and breast health education to women in underserved communities in the Sacramento valley region. Y-ME will conduct at least 25, A Day for You events between April 2008 and March 2009 screening at least 500 women. All programs are conducted with language services available. Will provide mammography services to at least 10 working poor women without insurance.

ENCOREplus Breast Cancer Program
This program educates women of all ages about the importance of and necessity for early detection in treating breast cancer. Low-income and uninsured or under-insured women 39 and younger are eligible for free breast exams, screening mammograms and breast ultrasounds. Emphasis is placed on medically underserved ethnic and racial minority populations, including African American, Asian/Pacific Islanders and Latinas. Other services provided include translation and weekly support and water therapy groups.
